Medical Records Release Policy

  1. All medical records to be released from South County Internal Medicine require an executed “Records Release” form or an authorization letter signed by the patient, their power of attorney or other authorized representative.
  2. Only those records specifically requested may be released.
  3. There is a fee applied to both the electronic and paper records. Pre-payment is required before records are released.
  4. No fee is required in the following situations:
    • The records are going to another physician’s office for continuation of care
    • The request for immunization records for school admission
    • The records are to support a claim or appeal under the Social Security Act
    • The records are to support a claim or appeal under any federal or state needs-based benefit program such as Medical Assistance, Rite Care, Temporary Disability or Unemployment Compensation
    • Records used for benefits in connection with a Civil Court Certification proceeding
    • Records are for up to three(3) pages of recent lab or test results to be used by the patient for personal use
  5. Records will be provided within thirty (30) days as required by
    law. A special handling fee of an additional $10.00 will be charged
    if the records must be delivered to the patient or authorized
    representative within forty (48) hours of the request.
